| Please choose the diagnostic test that you are having
performed at our facility. Information regarding the proper preparation
instructions will appear. If at any time you have questions about
the proper preparation for your exam, please call our center and ask
for the technologist. Also there is a list of forms that will need
to be filled out for the appropriate exam. If you print them and fill
them out, you can expedite your check-in time. Please leave the empty
box at the top of the forms blank, as we place a sticker with your
demographics in that spot. |
